(IF YOU HAVE COMPLETED THE ONLINE FORM, YOU DO NOT NEED TO ENTER YOUR DEMOGRAPHIC INFORMATION IN THE SHADED AREA.) ----- Simply write your child’s name and Date of Birth and sign the appropriate signature lines.

Your signature is required in 4 places on the registration form.

In addition to completing the registration form, there are several other forms that will be necessary for players to play in the DVHL and for the Wilkes-Barre Junior Pens organization.

Print and complete the necessary forms that are required by the DVHL and or USA Hockey. (all forms are provided in PDF format on the Forms Section of the registration process)

Prior to being rostered on any Junior Pens team all players will be required to register with USA Hockey as an “Ice Player”. (see USA Hockey web link below) Any USA Hockey registrations completed after April 1, 2010 will be effective immediately and be active until August 2011.

For those who played in the organization last year and registered with USA Hockey, your 2009-2010 registration will expire in August of 2010 and will need to register as a player for the 2010-2011 season.

Upon the conclusion of your registration you will be provided a bar coded registration confirmation. Please keep a copy for your records and provide a copy to the organization.
